Clinical evaluation of weight-loss drugs and rational nutritional indicators: Side effect analysis of orlistat and seven basic indicators of dietary composition
Thyroid hormones promote tissue catabolism and increase nerve excitability. They accelerate fat breakdown by increasing mitochondrial enzyme activity in fat cells, thereby raising the basal metabolic rate and achieving weight loss. However, this only provides temporary weight loss and can cause serious complications, such as hyperthyroidism, osteomalacia, and heart disease. The FDA has stated that thyroid drugs should not be used to treat obesity. Xenical, also known as orlistat capsules, is a weight-loss drug that inhibits gastrointestinal esterases.
Almost all dietary fats exist in the form of triglycerides, which are broken down and absorbed by gastrointestinal esterases. Xenical may bind to the active sites of gastrointestinal esterases, thereby blocking esterase activity and preventing approximately 30% of dietary fat from being digested and absorbed. The remaining 70% of dietary fat is absorbed by the body. Clinical studies have confirmed that the adverse reactions of Xenical are related to increased dietary fat intake, mainly including: oily spots, increased flatulence, defecation urgency, steatorrhea, and even fecal incontinence. These reactions usually appear in the early stages of treatment.
Common acute reactions include abdominal pain, abdominal discomfort, flatulence, and watery stools. Less common adverse reactions include upper respiratory tract infection, menstrual disorders, and anxiety. This medication is contraindicated in patients with chronic respiratory distress syndrome, cholestasis, or allergies. Safety studies in children under 18 years of age are lacking. Pregnant and lactating women should not take this medication. Rational nutrition refers to a diet containing sufficient quantities, a complete range of nutrients, and appropriate proportions, maintaining a dynamic balance with the body's consumption. Rational nutrition includes the following seven basic indicators:
Adequate and varied dietary intake: Generally, individuals engaged in light physical labor should consume approximately 20 different types of food, totaling about 1500g, daily. Reasonable composition of calorie sources: Grains should provide 60%–70% of calories, tubers 5%–10%, legumes 5%, and animal products 20%–25%. Reasonable ratio of calorie nutrients: The recommended ratio of carbohydrates, protein, and fat intake is 6.5:1:0.7.
A balanced calorie intake is essential: carbohydrates should provide 60%–70% of calories, fat 20%–25%, and protein 10%–15%. Protein sources should be well-balanced: 70% plant-based protein, 25% animal-based protein, and 5% legume protein, with high-quality protein comprising at least 30%. Fat sources should also be well-balanced: 60% plant-based fat, 40% animal-based fat, with saturated fatty acids accounting for less than 10% of total calories.
The recommended daily intake of various nutrients should meet the standard intake levels, which should be determined based on factors such as age, gender, and the intensity of one's occupation. The average daily intake of each nutrient over a 5-7 day period should not deviate by more than 10%. Adequate nutrition comes from a balanced diet.
